The nuchal translucency scan is a non-invasive ultrasound procedure usually performed on the abdomen area, although it may also be performed vaginally. This assessment is important as it allows the diagnosis of potential fetal complications including Down syndrome. Unless there are concerns about the pregnancy it is better to wait to have this scan after 7 weeks as the images are clearer for you to see and better for us to measure. If you come before 7 weeks the baby will be less than 1cm in size and so the scan sometimes has to be performed transvaginally placing the probe inside the vagina to enable us to see the baby well enough to measure and to see the heartbeat. Nuchal Translucency Ultrasound

A dating scan is an ultrasound examination which is performed in order to establish the gestational age of the pregnancy. Most dating scans are done with a trans-abdominal transducer and a fullish bladder. If the pregnancy is very early the gestation sac and fetus will not be big enough to see, so the transvaginal approach will give better pictures.

Dating scans are usually recommended if there is doubt about the validity of the last menstrual period. By 6 to 7 weeks gestation the fetus is clearly seen on trans-vaginal ultrasound and the heart beat can be seen at this early stage 90 to beats per minute under 6 to 7 weeks, then to beats per minute as the baby matures.

Protocol for assessment The gestational period must be 11 to 13 weeks and six days. The magnification of the image should be such that the fetal head and thorax occupy the whole image. A mid-sagittal view of the face should be obtained. This is defined by the presence of the echogenic tip of the nose and rectangular shape of the palate anteriorly, the translucent diencephalon in the centre and the nuchal membrane posteriorly. Minor deviations from the exact midline plane would cause non-visualization of the tip of the nose and visibility of the maxilla.

The ultrasound transducer should be held parallel to the direction of the nose and should be gently tilted from side to side to ensure that the nasal bone is seen separate from the nasal skin. The echogenicity of the nasal bone should be greater that the skin overlying it.

What to expect at your 12 week scan
